CASE MANAGEMENT PROGRAM

SERVICES

Mental Health Case Management assists adults and children with serious mental illness or emotional disorders in gaining access to needed medical, social, educational and other services.                                                                         

The goals of the program are to maximize an individual's ability to live independently and productively in the community.

ELIGIBILITY

Individuals with a serious mental health disorder in need of home based casework services.

No income restrictions and no cost for service.

 

LOCATION

Services are provided in the McKean County area.

 

PROGRAM STAFF                  

Caseworkers provide home and community based services.

Staff requirements are a Bachelor's Degree in Social Work, Psychology, Education, Criminal Justice or to be a registered nurse.
